Every year, a significant proportion of temperature-sensitive medicines in Nigeria are compromised before they reach the patient. The damage is usually invisible: a vial looks normal, the label is intact, and nobody notices anything wrong — until a treatment fails.
What breaks the cold chain
The failures are rarely dramatic. They happen in the gaps: a consignment left on a loading bay while paperwork is signed, a rider taking a detour to another pickup, a cooler box opened to check contents at a checkpoint, or an insulated bag reused after the ice pack has warmed.
Each of those small delays adds to cumulative heat exposure. Vaccines in particular tolerate very little of it.
What proper handling actually requires
First, planned routing. Every unnecessary stop is heat exposure. A cold-chain delivery should run point to point, with no intermediate transfers.
Second, validated containers. A cooler box that has never been tested for its hold time is not a cold chain — it is a hope. We test hold times and match container size to journey length.
Third, monitoring. Temperature should be logged for the duration of transit, not spot-checked at the end. The log should be available to the receiving facility, because they are the ones who need to trust the consignment.
Fourth, speed of handover. The cold chain does not end at the door. It ends when the product is transferred into the receiving facility's own cold storage. We wait for that to happen.
What this means for your pharmacy or clinic
If you receive temperature-sensitive stock, ask your logistics provider three questions: what is the container's tested hold time, is temperature logged throughout, and can I see the log? A provider who cannot answer those questions is not running a cold chain.
